Hathaway Dinwiddie requires all subcontractors seeking to bid projects greater than $50,000 to be prequalified. In order to prequalify, please complete the Contact Form to receive a prequalification invitation.
We may request submission of the following documents:
- Business Classification Certificate (if applicable)
- Contractors and City business licenses for California (if applicable)
- EMR verification letter showing Experience Modification Rates for the last 3 years
- OSHA 300A summary logs for work related injuries and illnesses for the last 3 years
- OSHA violations documentation for the last 3 years (if applicable)
- Injury Illness Prevention Program and/or Safety Manual
- Evidence of Insurance Certificate which must include Endorsements and Waivers
- Bonding Capacity and Bonding Rate verification letter
- Line of Credit Verification letter issued by your banking institution
- Current and prior year, audited or reviewed financial statements including footnotes
- Work Experience Reference List showing your company’s 3 largest projects
We do not require the following scopes of work to complete the prequalification process:
- Design and Engineering
- Material Suppliers/Vendors
- Planning and Surveying
- Testing, Adjusting, Balancing
